Saint Perpetua Catholic Church

To begin registration for faith formation please complete all of the steps below.

Classes will be limited to 18 students and class time will be confirmed once

all forms, payment and volunteer sign ups have been received.

Please note our First Eucharist preparation is a two-year process. 

• First Eucharist classes begin when the child is in 1st grade or older.

• In the second year of preparation the child will receive the Sacrament of First Eucharist.  

• If your child is in 2nd grade and just starting in our program please plan to have them attend Sacrament Year One classes.

• Only returning students may register for the Sacrament Year Two classes.

• If you are new to our parish and your child has attended faith formation at another parish please contact Kristine.

THE FOLLOWING CLASS TIMES ARE FULL:

2nd grade Sunday at 6:00 p.m.

4th grade Sunday at 6:00 p.m

Faith Formation page.

• All families are asked to volunteer for 4-5 hours of service throughout the year.

• Families who are unable to complete these hours are asked to include the $150 Buy-out fee with registration. 

• Please select a Sign up Genius to match your first choice class time.

Please do NOT return forms via email. All paperwork must be received with payment in order to process class selection.

If the class time is still available registration will be accepted after August 25 but closed by September 8.

4. Class time will be confirmed via email once steps 1-3 are complete and all paperwork and payment have been received. If you have any questions or need assistance selecting the best class or volunteer opportunity please contact Kristine at kristine@stperpetua.org.
